About this role
The Perinatal Case Manager supports maternal and infant health outcomes by providing comprehensive prenatal and early postpartum support services. This role integrates behavioral healthcare within the OB outpatient setting. As a licensed therapist, it focuses on case management, psychosocial counseling, client psychoeducation, and health promotion for healthy pregnancies and babies.
Day-to-day involves counseling patients during pregnancy and up to one year postpartum by identifying psychosocial disorders. Deliver psycho-education on perinatal mood and anxiety disorders in group or individual settings. Perform short-term counseling, crisis intervention, and maintain clinical documentation in Epic.
Coordinate with clinical, health education, community health staff, and other entities for service provision and referrals. Assist in implementing programs like Prenatal Plus and ROSES. Serve as liaison to community individuals and groups while providing in-service presentations to staff.
Offer clinical supervision to students in graduate education programs. Engage in psychosocial care-planning, goal-setting, monitoring, follow-up, and plan revision. This integrated care model ensures women access needed services for optimal outcomes.
Requirements
- Master's degree in Social Work from an accredited program
- Current valid LCSW license
- Three years experience as a Care Manager in an acute care setting
- At least five years of care management in an acute care setting (preferred)
- Care Management Certification (preferred)
Responsibilities

- Maintain clinical documentation in Epic
