Our technology-driven platform engages patients early and delivers coordinated virtual care throughout pregnancy, NICU stays, and postpartum.

Your North Star is to provide direct patient care and navigation delivering evidence-based perinatal mental health care founded in awareness, education, and prevention. As a clinical team member, you support whole-person emotional, mental, and behavioral care for pregnant people before, during, and after pregnancy. This part-time salaried role focuses on reducing disparities in access to high-quality care.

Reporting to the Director of Therapy, you conduct one-on-one virtual visits, screenings, psychosocial assessments, and triage urgent concerns. You establish individualized care plans, address barriers, and collaborate with multidisciplinary teams to meet patient needs. Documentation and escalation ensure seamless care coordination.

Requirements

  • Independently licensed as an LCSW, LMFT, LPC or LMHC for at least 5 years
  • Completed the NCMHCE and/or NCE
  • Have at least 120 hours of clinical supervision completed
  • Have 5-8 years of experience providing therapy services in a hospital and/or healthcare practice
  • Have experience with perinatal mental health care, including perinatal mood and anxiety disorders, substance abuse disorders, trauma-informed care
  • Enthusiastic for promotion of diversity, equity, and inclusion in all aspects of programs
  • Possess strong organizational skills and ability to prioritize multiple projects
  • Facile with email, spreadsheets, EHRs, and other software

Responsibilities

  • Provide one-on-one virtual care visits for Pomelo patients
  • Provide behavioral health and health related social needs screenings and appropriate interventions to pregnant and postpartum patients
  • Perform ongoing psychosocial assessment of patients, providing reassurance and building rapport
  • Establish individualized care plans to meet specific health needs based on evidence-based standards
  • Develop wellness goals by determining motivating factors for best possible health outcomes
  • Identify and address barriers identified by patients
  • Reduce care gaps by frequent engagements with patients
  • Triage patients with urgent screening results to appropriate services and collaborate with teams
